Are you scheduled for an exam for Social Security Disability Benefits?
Please make sure you have the following:
- Have a phone or computer with a camera and microphone
- Have secure internet
- Be in a private room with no one else around
- Have a picture ID and any medications you take
- Have your appointment letter
Once you have everything, please do the following:
- Below you will find a list of our providers. Please find the doctor who is on your appointment letter
- Click on “Informed Consent” below the doctor you are scheduled with
- You will be taken to a page that will explain the purpose of the exam. You will be asked for your signature to confirm that you understand and agree what you read.
